When correctly applied a system safety process can improve safety. However, inappropriate use of system safety methods can increase risk. Project examples will be provided to support the discussion of lessons learned for implementing the key steps in the system safety process. Example projects and accidents will illuminate: 1) how hazards can be misidentified, improperly assessed, and ineffectively controlled; and 2) how misapplication of surveillance and testing may provide a false sense of security for controlling risks. The topics should provide a lively discussion and audience participation is expected.
Upon completion, the participant will be able to:
• Discuss lessons learned from implementing system safety programs.
• Incorporate literature and materials provided by the CSB into an organization's program.
