Introduction:

Studies have shown that transcutaneous auricular vagal nerve stimulation (taVNS) can relieve the emotional and digestive symptoms of functional dyspepsia (FD) (Shi, 2024) (Zhu, 2021). But the brain mechanism remains unclear. The aim of this study was to explore the immediate brain function modulation effects of taVNS on FD by resting-state functional magnetic resonance (rs-fMRI).

Methods:

Thirty patients with FD and 47 healthy Controls (HCs) were enrolled in this study. The FD group received immediate (30 min) taVNS treatment and underwent rs-fMRI scan before and after the intervention, the HCs did not receive any intervention and underwent rs-fMRI scan after inclusion. Before treatment, amplitude of low frequency fluctuation (ALFF) and fractional ALFF (fLAFF) calculations were employed to compare FD group with HC. Subsequently, the differential brain regions were selected as regions of interest (ROI) to explore voxel-based functional connectivity (FC) changes after immediate taVNS treatment.

Results:

Compare with healthy subjects, patients with FD had higher levels of ALFF in left insula and lower levels of fALFF in right postcentral gyrus. The FC of left insula and left anterior cingulate (ACC) in the FD group were elevated after immediate taVNS treatment.

Conclusions:

In FD patients, the neural activity of insula and postcentral gyrus differ from healthy subjects. The insula and ACC might be the neural target of taVNS for the treatment of FD.
